茲申請小鼠診所服務而寄養於小鼠診所動物房之動物已實驗結束，委託貴單位以動物福利及人道方式代為犧牲處置，特以此書同意執行，日後若有異議，將自行負責。

Since my experiments tested in TMC were complete, I hereby authorize TMC to execute mice sacrifice following the animal welfare and humanity issues. I am totally responsible for this authorization and would not have any controversies with TMC later.
