A. Check Out

 

1. For animal check-out, please submit "Application Form for Mouse Hotel(Check out)".

2. If you would like TMC to assist sacrifice, please submit "Authorization Form for Animal Sacrifice".

   Please send required documents to mousehotel@tmc.sinica.edu.tw or fax to +886-2-7750-5505 by

   16:30 a working day ahead. For an urgent case, NTD100/cage will be charged additionally.

 

For example:
1. If the check-out/sacrifice date is Friday, please submit the document by 16:30 on Thursday.
2. If the check-out/sacrifice date is Monday, please submit the check in form by 16:30 on the previous                   Friday.

B. Notice

1. Please see "the revised routine for animal transfer"for reference to pick up your animals.
    When you arrived National Biotechnology Research Park, please dial +886-2-7750-5500#1883 or

    #1766 or #1772。

2. Animal pick-up time: 10:00-16:30, working day
